Abstract

Background: Smoke-free area (SFA) policies are critical public health interventions to reduce secondhand smoke exposure. Despite regulatory frameworks, implementation challenges persist in Indonesian healthcare facilities. Objective: This study evaluates the implementation of Bali Provincial Regulation No. 10 of 2011 concerning Smoke-Free Areas at Prof. dr. I Goesti Ngoerah Gde Ngoerah Regional General Hospital in Denpasar, Bali, using Edward III's policy implementation framework. Methods: A qualitative descriptive approach was employed with purposive sampling of six key informants, including hospital management, occupational health and safety (K3) team members, and supervisory unit personnel. Data were collected through in-depth interviews, direct observation, and document review. Analysis followed Edward III's four dimensions: communication, resources, disposition, and bureaucratic structure. Results: Communication efforts through socialization, prohibition signage, and spot inspections were implemented but remained suboptimal. Resources including the K3 team and supervisory units were available, yet public and visitor compliance remained low. Disposition was favorable with a well-established supervisory task force. Bureaucratic structure functioned adequately with enforcement mechanisms including warnings and sanctions. Key barriers included insufficient public awareness, cultural acceptance of smoking, and inadequate loudspeaker facilities for announcements. Conclusion: While structural and dispositional elements are in place, communication and resource mobilization require strengthening to enhance compliance. Findings align with broader Indonesian experiences and suggest the need for intensified socialization, cultural sensitivity, and enhanced enforcement mechanisms.
